I, the undersigned parent/legal guardian, give permission for my child(ren) named above to attend and participate in the FWC Youth Lock-In, including overnight stay under the supervision and care of Freedom Word Church’s approved chaperones and leaders.

I understand that:
  • All activities will take place under the guidance of responsible adults and leaders.
  • My child(ren) is expected to follow all rules, respect leaders, and participate appropriately.
  • In the event of illness, injury, or emergency, I will be contacted at the phone number(s) provided below. If I cannot be reached, I authorize the leaders to secure medical care deemed necessary for my child(ren).
  • Freedom Word Church, its staff, and volunteers are not responsible for lost or damaged personal items.
  • I release Freedom Word Church, its staff, and volunteers from any liability for injuries or damages incurred during participation, except in cases of gross negligence or willful misconduct.
